APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company

-Docket No. 50-354 Hope Creek Unit 1-flicense No. CP?R-120 As a result' of the -inspection conducted on December 1,1980 -- January 4,1981 and in accordance with~ Interim Enforcement Policy, 45 FR 66754'(October 7, 1980),

the following violation was icentified:

10 CFR 50, Appendix B, Criterion XIII states, in part, that:

" Measures shall be. established to control... cleaning and preservation of material'and equipment in accordance with work and inspection instructions to prevent da.;. age or deterioration...".

.Section 16.4.13 of Chapter 16 of the PSAR states, in part, that:

" Procedures for equipment and system... cleanliness control conform with the appropriate

requirements of Regulatory Guide 1.37...".

Paragraph 4 of ANSI N45.2.1-1973 which is endorsed by Regulatory Guide 1.37 states, in part, that: " Inspections, examinations...shall be performed immediately prior to installation to determine the cleanliness of ths item.

If contaminants are detected, they shall be removed...".

Contrary to the above, as of December 18, 1980, foreign matter consisting of wood and steel rod was visually observed to be contained inside the 24" HPCI turbine steam discharge line located in the suppression chamber.

Final closure welds had been made by the installing subcontractor. This is considered a third item of noncompliance relative to piping internal clean-liness.

This is a Severity Level V Violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201,'Public Service Electric and Gas Company is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explar.ation in reph, f r.cluding:

(1) the corrective steps which have been taken and the results achieved; (2) i corrective stens which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
