Re: Renewal of License #SNM-1629

Dear Str. Wade:

Our special Nuclear Materials License No. SNM-1629, expires on May 31, 1981 and we would like to have that license renewed.

Please accept this letter as an application for renewal. Plutonium (principal radionuclide Pu-238) in sealed source forms will be used in accordance with our pre-itous license application received by the Nuclear Regulatory Commission on April 12, 1976.

We are requesting a maximum possession limit of 15 grams. The scaled sources will be used by the physicians listed on the license issued on May 5, 1976.

Seven copies and a check for $150 as renewal fee have been attached. We hope the application is satisfactory.
