Dear Mr. Heishman:
This letter constitutes the written statement reqaired in reply to the subject February 1981 NRC Inspection Report (Reference 1) which we received April 13, 1981. Appendix A, " Notice of Violation," listed one item of noncompliance.
ENFORCEMENT ITEM Infraction:
LACBWR's Technical Specification 6.8.1 required procedures covering safety related activities listed in Regulatory Guide 1.33 to be established and implemented.
Contrary to the above, on February 1,1981, a plant operator failed to properly implement a switching procedure causing a loss of offeite power.
DPC RESPONSE Corrective Action Taken and Results Achieved Although the operator executing the written switching order was qualified to perfonn the switching, he incorrectly read the label on the switch handle and consequently opened 69 KV air break disconnect 25NA4 under load. This action caused a plant trip.

Heishman Chief, Reactor Operations and LAC-7488 Nuclear Support Branch April 24, 1981 011 circuit breaker 25NB4 was then opened remotely from the Control Room and switch 25NA4 was manually closed. Offsite power was restored within 15 minutes by restoring 25NB4. Following these actions, the correct switch 25NA1 was opened without incident.
The actions of the switchyard operator were reviewed promptly with him by the Operations Supervisor, who was at the plant at the time of the incident. The operator was later interviewed in a meeting which included the Plant Superintendent, Assistant Plant Superinteraienc, Operations Engineer, Assistant to Operations Supervisor, and the Operator's Shift Supervisor.
Corrective Action Taken to Avoid Further Noncompliance Practical switchyard training and retraining is being provided to LACBWR Operators by the DPC Superintendent of Transmission and the Dispatcher Supervisor. The majority of this training has been completed. The last session is scheduled for April 21, 1981. Also, whenever practicable, outside switching orders will be executed in the presence of a second person to provide a double check.
It is our opinion that the additional training and the use of a second person to perform a verification check should result in improved performance of switching orders.

The number of personnel errors which occurred at the beginning of this yeae also greatly concerned DPC management. Additional training, such as the switchyard training has been provided to the operators. All members of the Operations Department have reviewed the reports on the recent instances of personnel error. This dissemination of operating experience should help prevent reoccurrence of similar incidents.
Plant management has engaged in interviews with several operators and their Shift Supervisors to convey management's concern with operator performance. We are confident that these actions should help minimize future personnel errors.
