Dear Mr. Grier:
IE Bulletin 80-25 describes several instances of malfunctioning of two-stage, pilot-operated, Target Rock safety-relief valves (SRV).
Briefly, the malfunctions were due to solenoid actuator failure (excess Loc-Tite in solenoid plunger), foreign material lodged in the main valve seat and excessive pneumatic supply pressure to the solenoid actuator.
The Shoreham Nuclear Power Station is equipped with identical valves.
As directed by the bulletin, the following actions have been or are being undertaken:
1)
We will initiate appropriate quality control procedures to assure inspection of the solenoid actuator for excess Loc-Tite in accordance with manufacturer's recommendations.
2)
Plant Staff and Startup have been notified of the require-ment to remove the entire valve from service for disassembly, inspection and adjustment if it fails to function as designed.
3)
A review of our SRV pneumatic supply overpressure protection is nearly complete.
Any required modifications will be implemented prior to Fuel Load.
Operating procedures will be prepared to address operator response to overpressure conditions.
In response to your request for information to assist in evaluating the value/ impact of each bulletin, we estimate that approximately 48 engineering manhours have been expended in responding to this bulletin.
