10CFR50.55(e) Report on Agastat Relay Bases (#23)
Dear Mr. Keppler:
Detroit Edison's final report on the Agastat Relay Base problem was subnitted to you on November 3,1980. Subsequent werk in this area has indicated that a change in the action indicated on that report will be necessary. We are, therefore, subnitting this supp1 ment to the above referenced final report.
The problen was originally reported to Mr. R. Knop by Mr., H.A. wad.er in a telephone call Novatber 9, 1979. It had been noted in perfoming checks on the Primary Containnent Monitoring System that scoe circuits through the Agastat relays were open when they should have been closed.. Investigation revealed that pins in the relay mounting brackets connecting to the relays were loose and if a good alignment of relay connector pins to mounting bracket terminal was not obtained, then the mounting bracket terminal was pushed away frcm the relay resulting in an open or intemittently open circuit. At the time of the report, the problem had been noted only in the two panels of the Containment Monitoring Systms (panels H11-P914 and H11-P915)
Subsequent investigation has revealed that this type relay base is also used in five e.Jditional panels which were supplied by General Electric.
Detroit Edison has ordered a different nodel of relay nounting base for panels H11-P914 and H11-P915 which will replace all forty bases ncw in these panels. General Electric subnitted a proposal to modify the relay bases in the five G.E. supplied panels to eliminate the pin push out problem. This proposal was evaluated by Detroit Edison Engineering, and was accepted with slight modifications. However, subsequent problems with this method indicated the need for additional evaluation. As a result Detroit Edison has decided to replace the Agastat relay base in the five G.E. panels as well as those in the Containment Monitoring System. Purchase orders for the required material are being prepared and the work will be acccuplished as construction schedules allcw.
New installations or modifications to Agastat circuits of this type will I
incorporate the rar style relay nounting.
