Docket No. 99900349/80-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on September 22-26, 1980, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:

A.

Corner & Lada Co., Inc., (C&L) letters of corrective action dated January 9 and February 6, 1980, addressed their review of all product design check lists and manufacturing drawings, and the resultant, necessary corrections.

The measures taken to preclude recurrence, were to place the Product Design Files and all original manufacturing drawings into the main Quality Assur-ance file thus, putting them under complete QA control.

Specification ST-150 requires all drawings to be checked by a competent person other than the person who produced the original drawing, the Engineering Manager, and the QA Manager.

It also states that this will be accomplished by initialing and dating the drawing and the Product Design Check List.

In the casa of drawing revisions, the same review and approval cycle will be performed, utilizing the Document / Drawing Change Request (DDCR) and the Product Design Check List (PDCL).

After this has been acccmplished, revised drawings will be forwarded to Manu-facturing and Quality Assurance with a Drawing Change Notice (DCN).

Contrary to tae above, the measures taken to preclude recurrence have not been effective.

C&L is still not conforming to the requirements of ST-150, as evidenced by a review of 10 drawings, all dated after their corrective action was completed, in which 8 of the drawings either did not have approvals, DDCRs, PDCLs or a DCN.

B.

Criterion V of Appendix B to 10 CFR 50 states,

" Activities affecting quality shall be prescribed by documented instructions, i

i procedures, or drawings, or a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

QA Manual Section QN 3.1, paragraph 3.A. states, "All material shall ce inspected at source or shall be inspected upon receipt for conformance to i

the drilling and reaming of sway strut paddles, which is to be accomplished l

in accordance with Drawing A3217, Revision 1.

I Drawing A3217 specifies reamed hole tolerances of.811

.812 for the size 1 strut, and 1.248 - 1.249 for the size 2/3 strut.

2 Contrary to the above, dimensional inspection of reamed holes on a sample of 5 - size 1, and 10 - size 2/3 sway strut paddles revealed that none met the drawing requirements.
