Dear Sir:

Pursuan t to receipt of your correspondence regarding the results of the subject inspection, we offer the following reply:

N' C Notice of Violation (81-01-01) 10CFR50, Appendix B, Criterion V s tates , in part, that: " Activities affecting quality shall be . .. accomplished in accordance with (the prescribed) ins tructions , procedures, or drawings."

The Seabrook Station PSAR for Units 1 and 2 states , in part, in paragraph 17.1.5 tha t: "Each organization is required to perform their respective quality related activities covered by this program in accordance with documen ted ins tructions , procedures , or drawings ."

Perini Power Cons tructors Field Civil Cons truction Procedure FCCP-156, Revision 2, s tates in accordance with AWS D1.1 requirements in paragraph 12.2 that: " Parts to be joined by a tee or fillet weld shall be brought into as close contact as is practicable. ...If the separation is greater than 1/16",

the leg of the fillet weld shall be increased by the amount of separation."

UE&C Drawing F101550, Revision 3, indicates that the welding of a W10 structural beam (Cives Mark No. 135AG) to the flange of a W24 beam requires a 5/16" fillet weld.

Con trary to the above, on October 7,1980, the structural fillet weld connecting piece No. 135AG was signed off as acceptable by Perini QA personnel despite the fact that a 5/'22" gap exis ted between con tact surfaces and the leg of the 5/16" fillet weld nad not been increased by a corresponding 5/32" in size.

Response

iorrective Action Taken and Results Achieved Perini issued NCR NO. 1447 to document the identified deficiency;and required that the weld size be increased to offset the 3/16" gap. The inspector who accepted the welds has been retrained in structural weld inspection criteria.

The welder who ' accomplished the de ficient joint has been re-trained regarding join t gap requirements.

Additionally, Perini, on a random basis , reinspected fifty (50) welds that had previously been accepted by the subject inspector. Included in the sample were nine (9) welds with increased leg size. All welds checked were found to be satisfactory.

Full compliance was achieved on March 11, 1980.

NRC Notice of Violation: (81-01-02) 10CFR50, Appendix B, Criterion V, s tates , in part, that: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings .. . (which shall include appropriate quan titative or qualitative acceptance criteria. . . "

The Seabrook Station PSAR s tates , in part, in paragraph 17.1.5 that: "Each organiza tion is required to perform their respective quality related activities covered by this program in accordance with documen ted ins tructions ,

procedures, or drawings (which) shall include appropriate quantitative or qualitative acceptance crite ria . . . "

Con trary to the above, as of January 28, 1981, certain field welds (FW39 and

41) on hanger 1226-A-17 had been welded as fillet welds connecting structural pieces at angles greater than 1050, despite the fact that the applicable drawing (Pullman Power Products drawing for Hanger No. 1226-A-17, Revis ion 6A) specified no criteria for the welding of fillet welds at angles greater than 1050 This resulted in the exis tence of welds of ques tionable throat size and strength.

This is a Severity Level V Violation (Supplemen t II) .

Response

Corrective Action Taken and Results Achieved The contractor (Pullman-Higgins) immediately stopped the welding operation on all skewed welds (welds with dihedral angles exceeding 1050 or less than 600). Based upon UE&C Engineering Change Approval No. 25-0327A, Pullman-Higgins developed Field Instruction No. FI-103 which details methods for the ins tallation and inspection of skewed welds. In addition, Pullman-Higgins procedure IX-6 " Hanger Installation and Inspection Procedure" is being revised to include skewed fillet weld ins tallation criteria.

Units,d Statas Nuclesr Ragulatory Commission March 17,1981 Offics of Inspection and Enforcsment Piga 2 Training sessions have been held with craft and QA personnel at which time skewed weld fabrication and inspection requirements were clarified.

Concurren tly, Pullman-Higgins is reviewing other drawings to assure that clear ins tructions are included. A reinspection is also being performed of previously installed skewed welds. Suspect welds will be identified and receive engineering evaluation.

The UE&C construction canager transmitted a letter to all applicable site contractors which includes criteria for installing and inspecting skewed welds. YAEC QA, during its surveillance activities will assure compliance with project welding requirements.

Full compliance will be achieved by March 31, 1981.
