VTVYS1 05000271 LER 81-09/3L Event Description and Probable Consecuences:

During the weekly environmental air sa=ple collection performed on 3/17/81 and 4/7/81, it was discovered that continuous sa ples were not being drawn at stations AT 1.3, AT 1.5 and AT 1.4.

Technical Specificacicus Table 3.9.1 requires that continuous air sc=pling be perfor=ed. During the above periods, sonitored plant stack release levels were nor=al and past experience has shown that these levels are undetectable by environmental air sampling. Therefore, these incidents were of no consequence, or potential censequence. to the health and safety of the public.

Similar occurrences were reported as RO 80-21/3L and R0 81-05/3L.

Cause Descriptien and Corrective Actions:

Newly purchased pumps were placed into operation at all staticus on 3/10/81. Upon examination of the above three failed pumps, it was deter-sined by the maintenance electrician that an "open" in the windings of each motor caused the pump failures. In addition, one pump had a misaligned carbon stem guide which produced excessive friction in the pump. Replace-ment pumps were installed at each station and the failed pumps were returned to the vendor for repair or replacement.

In the future, new pumps will be bench tested prior to being installed in the field.
