This certificate is issued to satisfy Sections 173.393a,173.394.173.395, and 173.396 of the Departrnent of Transportation Hazardous Materials Regulations (49 CFR 170-189 and 14 CFR 103) and Sections 146-19-10a and 146-19-100 of the Department of Transportation Dangerous Cargoes Regulations (46 CFR 146-149), as amended.

An overpack that provides impact and thermal protection for a teletherapy head assembly.
The cubical overpack consists of laminated 2x4 hardwood maple panels bolted together and covered with 16 gage steel panels.

Reinforcing steel straps and angles are welded together and spaced to limit the openings between them to less than 6 inches.
Skid runners are provided to facilitate fork lift usage.
Dimensions of the overpack are 36"x42"x40.5" with a maximum gross weight of 4000 pounds.

(b) Contents (1) Type and form of material Co-60 sealed sources that meet the requirements of special form as defined in 10 CFR 571.4(o).
The sources are to be packaged in a secondary inner container (teletherapy head) as described in Item 5(a)(3).
(2) Maximum quantity of material per package 13,680 curies, with a radioactive decay heat load not to exceed 200 watts.

The package authorized by this certificate is hereby approved for use under the general license provisions of 10 CFR 971.12(b).
