Dear Mr. Keppler:

On July 17, 1980, Mr. T. D. Geib of Public Service Company of Indiana .

Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). The support steel supplier for the Nuclear Steam Supply System, Lakeside Bridge and Steel Company, notified PSI that certain radiographs of the steam generator upper lateral support embedments did not meet the density requirements of Section V of the ASME BSPV Code.

As a result, acceptable radiographs of these e mbedments did not exist and their quality was indeterminate. In an interim report dated August 13, 1980, PSI identified several additional embedments and pipe restraints whose radiographs also did not meet the density requirements of the Code. PSI included these items within the scope of the existing concern.

Since that time, Lakesit Bridge and Steel Company has reexamined all the material in question. During the reexamination, certain pieces were dis-covered to contain weld deficiencies. Had these weld deficiencies remained uncorrected, failure of a weld might result if a support were f:o be sub-jected to the extreme loadings which might occur during certain design basis events.

g Lakeside Bridge and Steel Company has repaired and reexamined the welds cor.taining deficiencies in accordance with approved procedures. PSI's j architect-engineer, Sargent & Lundy, has reviewe' these reex m inations and found them acceptable and the material in questio.. to meet specification requirements. To prevent recurrence of this problem, Lakeside Bridge and Steel radiographic film viewing procedures have been revised.to incorporate double viewing to detect weld aeficiencies where film densicy requires.
