STN 50-498, STN 50-499 Tnird Interim Report Cnneerning Material Control of ASTM A-325 Structural Steel Bolts On June 17, 1980, pursuant to 10CFR50.55(e), Houston Lighting & Power Corapany notified your office of a potentially reportable deficiency concerning material control of ASTM A-325 Structural Steel Bolts.
Interim Reports on this subject were submitted to your office on July 17, 1980 and August 22, 1980.
The attachment to this letter constitutes the Third Interim Report concerr.ing this deficiency.
An update to the status of this concern will be submitted to ycar office by November 7,1980.

ATTACHMENT THIRD INTERIM REPORT CONCERNING MATERI AL CONTROL OF ASTM-A325 STRUCTURAL STEEL BOLTS September 23, 1980 DESCRIPTION OF DEFICIENCY At the South Texas Project Electric Generating Station (STPEGS) an investigation of an apparent premature inventory shortage of Category I Structural Steel Bolts revealed inadequate material control practices in effect, and incomplete material traceability documentation as provided by one Category I bolt supplier.
Imoroper segregation and identification of Category I and non-Category I structural steel bolts, both of ASTM A-325 high strength steel, has allowed bolts procured under a non-Category I specification to be released for Category I applications. Most of these bolts were supplied, however, with Certified Material Test Reports (CMTR's) which would allow the material traceability required for Category I appli-cations. This deficiency is documented in Nonconformance Report (NCR)
S-4377.
Previous Interim Reports have identified four Action Items in progress, in order to resolve this potentially reportable deficiency.
Action Items and status of these items are as follows:
ACTION ITEM (1)
Release of Structural Steel Bolts for Category I applications has been halted under Stop Work #S-22 until full material traceability is established.
Status-Partial release on Stop Work #22 allows installation of bolts for which CMTR's have been provided.
Bolts installed under this partial release are color coded for ready identification.
ACTION ITEM (2)
Documentation for installed bolts in Category I applications is being identified and located.
Status-All bolt supplier's have been contacted and have verified they can and will provide CMTR's for all bolts delivered.
Approximately 60%
of the required CMTR's have been received, with the remainder expected shortly.
ACTION ITEM (3)
Material Control methods and procedures are being evaluated for necessary modifications and improvements.
3tatus-All used and galvanized bolts are being scrapped so that only certified Category I structural bolts will be on site.
All new orders are purchased as Category I bolts.
The possibilit" of further installation of non-Category I bolts has therefore been elimit d.
General Consturction Procedure A040KPGCP-22 (General Material R.W og and Storage) provided adeauate procedural requirements to prevent
'mproper handling of the bolts.
However, these requirements were not thorou allowed.
Letters have been
Page 2 issued which direct warehouse personnel not to issue raterial unless requisitions are properly completed.
Requisition completion is now to additionally include a material control autnorized signature.
Also, all warehouse personnel have received formal training on proper preparation of material requisitions to avoid acceotance and/or processino of naterial requistions which have not been properly prepared.
ACTION ITEM (4)
Documentation and Quality requirements contained in Category I Structural Steel and Miscellaneous Steel specifications are being reassessed to determine the adecuacy of the bolts installed.
Status-Complete.
CMTR's will be furnished by all suppliers for previously purchased bolts and all future purchases will recuire CMTR's prior to receiving.
RECURRENCE CONTROL The corrective actions stated above will preclude any recurrence of deficiencies concerning material control.
SAFETY EVALUATION There is no indication of any hardware problem which would lead to a potential safety ha:ard.
Upon receipt of the remaining CMTR's for previously installed bolts, the necessary documentation will be available to meet Category I requirements of full material traceability for these ASTM A-325 structural steel bolts.
