(V Appendix A NOTICE OF VIOLATION Toledo Edison Company Docket No. 50-346 As a result of the inspection conducted on November 12-14, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
Criterion XII of 10 CFR Part 50, Appendix B, states in part that " Measures shall be established to assure that instruments used in activities affecting quality are properly calibrated at specified periods to maintain accuracy within necessary limits." Quality Assurance Procedure 2120, Revision 2,
" Control of Measuring and Test Equipment," requires that written procedures be provided to accomplish the above.
Contrary to the above, calibration procedures were not provided to assure accuracy of the chart speed of the Brush recorders used in Cycle 2 control rod drop tests (Surveillance Test ST 5013.02, Revision 3, " Control Rod Assembly Test").
Consequently, the recorder chart speeds used in verifying control rod drop times for Cycle 2 were not calibrated.
This is a Severity Level VI violation (Supplement I.F.)
Pursuant to the provisions of 10 CFR 2.201, Toledo Edison Company is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:
- 1) corrective steps which have been taken and the results achieved; 2) cor-rective steps which will be taken to avoid further noncompliance; and
- 3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or af firmation.
