APPENDIX A Notice of Violation Gulf States Utilities Company Docket No. 50-458 CPPR-145 As a result of the inspection conducted on November 18-21, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality be prescribed by documented procedures of a type appropriate to the circumstances, and shall be accomplished in accordance with these procedures.

a.

Stone & Webster (S&W) Specification No. 210.370, Revision 5, " Placing Concrete and Reinforcing Steel," on page 1-44, states " Scaffolding, lighting, communication equipment, and other mechanical aides that can improve the steel and concrete placement shall be used wherever practicable."

Contrary to the above:

The IE Inspector observed that, during concrete placement No. SC4-W-85-7 (a3'-6"extgriorradfuswallintheNo.1StandbyCoolingTower,between azimuths 150 and 180, from elevation 64'-6" to 84'-6") no lighting was utilized to provide visibility at the bottom of the form.

During that portion of the placement witnessed by the IE Inspector, the S&W QC Inspector was observed to be performing his inspection from the top of the formwork.

In addition, the IE Inspector verified that the S&W Quality Control Inspector was not in possession of a flashlight.

Available light was such that the bottom of the form was dark and not visible.

This is a Severity Level V Violation (Supplement II, paragraph E).

Stone & Webster Quality Assurance Directive No. 10.2, Revision B,

" Inspection of Batch Plant, Concrete, Reinforcing Steel, and Grout,"

paragraph 5.1.3 states, " Water additions shall be checked to assure the amount prescribed in Reference 2.2 is not exceeded."

Reference 2.2, S&W Specification No. 210.350, " Specification for Mixing and Delivering Concrete," requires a maximum water to cement ratio of 0.394 for the applicable mix (H3).

Contrary to the above:

The IE Inspector observed that, during concrete placement No. SC4-W-85-7, batch ticket No. 14335 indicated that the addition of 10 gallons of water was allowed; whereas 12 gallons were actually added.

Computation of the actual water to cement ratio, from the information on the batch ticket, resulted in a value of 0.397.

The concrete truck was not rejected and the concrete was placed.

This is a Severity Level V Violation (Supplement II, paragraph E).

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities Company is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
