APPENDIX A NOTICE OF VIOLATION License No. DPR-77 Tennessee Valley Authority Sequoyah Unit 1 Based on the NRC inspection September 1-30, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indi-cated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Technical Specification 6.8, written procedures and changes thereto shall be established, approved, and implemented. Administrative Instruction AI-4 states that only controlled copies of procedures shall be used to perform work and that copies shall be located in specified locations.
Contrary to the above, on September 12, 1980, a controlled copy of System Operating Instruction SOI-3.2 " Auxiliary Feedwater System" was improperly located and available for use in the turbine driven Auxiliary Feedwater Pump Room. Additionally the procedure had several unapproved changes made to it.
