Dear Mr. Keppler:

On August 13, 1980, Mr. T. D. Geib of Public Service Company of Indiana Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). During a receipt inspection reverification of auxiliary steel performed by the Cherne Contracting Corporation (CCC) several deficiencies were identified. A number of pieces of auxiliary steel, steel used to support pipe hangers, had undercut welds, undersi o fillet welds and incorrect weld contour, pinholes or drawing non-compliances.

While this steel is conservatively designed, failure of some pieces might result if subjected to the extreme loadings which might occur during certain design basis events.

As a result of the deficiencies discovered during reverification inspection, CCC undertook a 100% reinspection of their auxiliary steel. The reinspection revealed additional deficiencies. PSI has examined some of these deficiencies and determined that the structural integrity of the support steel was not l

impaired for those pieces examined. However, due to the number of' deficiencies j generated during the 100% reinspection of auxiliary steel, it would require.

an extensive evaluation to determine if all pieces of auxiliary steel were l adequate to perform their intended function. Therefore all affected pieces of auxiliary steel will be reworked as necessary in accordance with approved procedures.

Construction CCC has strengthened the inspection practices used in their receipt inspection program. It is anticipated that this strengthening of CCC's program will prevent problems similar to this'one from occurring in the future.
