Docket No. 50-445/80-20 50-446/80-20 Appendix A NOTICE OF VIOLATICN 3ased on the results of the NRC inspection conductea on September 17, 1980, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Pemits No. CPPR-126 and 127 as indicated below:

Unsuitable Weld Surface Condition As Recuired By Macnetic Particle Test Procedures 10 CFR 50, Appendix B, Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings."

Subcontract 35-1195-0585 with Chicago Bridge & Iron Company states that Specification 2323-CDS-4 (Revision 1) shall govern the work involved except as modified by the notes appearing on Drawing 2323-51-0680 (Revision 2).

Note 7.2 on Drawing 2323-51-0680 requires, in addition to the ASME Code required visual inspection of welds, a magnetic particle or liquid penetrant examination be performed on each weld and the adjacent base metal.

Paragraph 13.1 -of the referenced ~ subcontract states that all quality activities are to be accomplished by the subcontractor in accordance with his approved Quality Assurance Manual.

The Chicago Bridge & Iron (CB&I) QA Manual associated with the above subcontract provides procedures and acceptance criteria in documents MT-3 and MT-4 for magnetic particle examination and PT-5 for liquid penetrant examination.

Contrary to the above:

Quality activities were not accomplished by the subcontractor in accordance with his approved Quality Assurance Manual, in that, on September 17, 1980, the NRC Resident Reactor Inscector, in conjunction with other NRC inspectors, selectively examined four of the ninety-five comoonents received at tne site under Subcontract 35-1195-0585 and noted discrepant conditions.

The exami-nation, conducted in accordance with Procedures MT-3 and MT-4, resealed indications that were considered unacceptable in accordance with the criteria contained within thereferenced CB&I procedures.

Specifically, the welds in components 32-A-1, 34-A-1, 35-A-1 and 36-A-3 were not sufficiently free from coarse ripples, grooves, overlap, abrupt ridges and valleys to render the surface suitable for ::rcoer interpretation of the required magnetic particle or liquid penetrant examination.

This is an infraction.
