Appendix A NOTICE OF VIOLATION 111inois Power Company Docket No. 50-461 4
Based on an inspection conducted on September 15-19, 1980, it appears that certain af.your activities were in noncompliance with NRC require-ments, as noted below. This item is an infraction.
10 CFR 50, Appendix B, Criterion V requires, in part, that " Activities affecting quality shall be prescribed by documented instructions, pro-cedures, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures..."
The Illinois Power Company PSAR, Section 17.1.5.1 states in part, that,
" Requirements for the use of instructions, procedures, sketches, drawings, and related material to control activities which may affect quality are generated and implemented within using organization.. Formal procedures, instructions, and drawings shall be developed and used as appropriate, for the activities affecting quality".
Baldwin Associates Technical Service Instruction BTS 402, r.ragraph 6.2.1, states that, "The welder shall have the pink copy of the " Field Requisition - Welding Material" Form BTSF-030 available for the Technical Services Welding Technical / Inspector to transcribe the heat / lot number, and welder's symbol to the documentation form relating to that weldment of the traveler, and also enter traveler information on the pink copy, sign and date it".
Contrary to the above, activities affecting quality were not in accordance -
with the appropriate instructions, in that, completed travelers did not contain the welders identification and the weld material heat / lot number as required by BTS 402.
