O APPENDIX A NOTICE C/ VIOLATION Northeast % clear Power Company Docket No. 50-245 This refers to the investigation conducted by representatives of the Region I (Philadelphia) office at Millstone Unit 1 at Waterford, CT., corporate offices at Berlin, CT., and your contractor's offices at Waltham, MA. of activities authorized by NRC License No. OPR-21.
During the investigation conducted on January 3-5, 1980, the following apparent item of noncompliance was identified.
10 CFR 50, Appendix B, Criterion, XVI, states in part:
" Measures shall be-established to assure that conditions adverse to quality, such as fail m s, malfunctions, deficiencies, deviations, defective material and equipment.,
and nonconformances are promptly identified and corrected.
In case of significant conditions adverse to quality...the cause of the condition, and the corrective action taken shall be documented and reported to appro-priate levels of management.
Contrary to the above, as of January 3, 1980, measures had not been esta-blished to assure that conditions adverse to quality were adequately iden-tified, documented, evaluated and corrected.
The following are examples:
the cracks in concrete of the drywell wall in the vicinity of penetration X-10A were not properly and adequately evaluated; and, the repairs performed on the loosened anchor bolts in pipe supports of the Isolation Condenser System were not properly documented.
This is an infraction.
