This refers to an Operational Safety Team Inspection (OSTI) conducted by

Messrs. W. L. Forney, M. A. Ring, P. D. Brochman, B. Drouin, S. Ray,

W. G. Rogers and J. A. Gavula of this office,(Ms. A. B. Sutthoff, a consultant

for the Office of Nuclear Reactor Regulation NRR),andJ.B.Hickmanand

Dr. M. C. NcCoy of NRR, on September 25, 1989, through October 3. 1989, of

activities at Clinton Power Station authorized by NRC Operating License

The enclosed copy of our inspection report identifies areas examined during

'

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representstive records, observations, and

interviews with personnel.

No violations of NRC requirements were identified during the course of this

inspection.

The purpose of this inspection was to verify that the Clinton plant was being

operated safely in conformance with regulatory requirements and that the

organizations that control and support plant operation were functioning

effectively to ensure safe operation. While no violations are being issued

with this report, several concerns were identified which we believe warrant

your attention.

The concerns include the following: poor material condition

of Balance of Plant (BOP) equipment, unreliable condition of certain important

to safety equipment, excessive burden on operating personnel, frequent use of

'

interim fixes, a developing culture of "getting around" specifications and

procedures, and slow involvement of engineering in solving operational

problems.

In addition, the OSTI noted concerns in areas which do not have direct

regulatory or safety correlation involving morale and perceptions of poor

communications between Clinton Powor Station management and the Clinton staff.

These concerns were also shared with you at the exit interview to provide
