Hospital authorized by NRC Byproduct Material Licenses No. 12-09239-01, No. 12-09239-02 and NRC Special Nuclear Materials License No. SNM-1676 and to the discussion of our findings with you and others of your staff at the conclusion of the inspection.

The inspection was an examination of activities conducted 'nder your u

license as they relate to radiation safety and to compliance with the Commission's rules and regulatiens and with the conditions of your license.

The inspection consisted of a selective examins. tion of procedures and representative recorda, observations, independent measurements, and j

interviews with personnel.

i In addition to the above areas, the inspectors examined actions described in your letters dated December 3 and 6, 1976, regarding apparent items of noncompliance found during our October 29, 1976, inspection. We have no further questions regarding these matters.

This inspection also included c measurement of the output of your tele-therapy equipment. Our m nsurement was made using a Victoreen Model 370

{

Condenser R-Meter with Model 621 chamber, calibrated by the National Bureau of Standards Laboratory. The source-to-chamber distance was 80.0 centimeters and the field size used was ten by ten centimeters. After applying standard correction factors fcr temperature, pressure, timer error, and roentgen-to-rad conversion, our measurement of the output of your teletherapy equipment was 90.8 rads per minute at the point of maximum buildup in a water miniphantom. The most recent monthly spot check of your teletherapy equipment, conducted on February 3, 1981, reported a measured output for the same exposure parameters of 92.3 rads per minute. Applying a correction face r to account for 30 days decay, the value calculated from the February 13, 1981, spot check becomes 91.3 rads per minute.

i Based on the close agreement in measured output values, we feel the actual output of your teletherapy equipment to be within established guidelines of 15 percent.

l During this inspection, certain of your activities corducted under License No. 12-09239-01 appeared to be in noncompliance with NRC requirements as described in'the enclosed Appendix A and a written response is required.

No items of noncompliance were identified for Licensee No. 12-09239-02 and No. SNM-1676.
