Appendix A NOTICE OF VIOLATION Roosevelt Memorial Hospital License No. 12-14860-01 As a result of the inspection conducted on April 16, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

1.

License Condition No. 14 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated December 10, 1976, and letter dated March 25, 1977.

The above referenced application states the procedures described in Item No. 14 will be followed. Item No. 14 resaires all packages con-taining byproduct material received in the Nuclear Medicine Department be surveyed and the results recorded.

Contrary to the above requirement, it was determined through statements by licensee representatives and the NRC inspectors review of records that this requirement is not being met.

Specifically, results of package surveys received in the Nuclear Medicine Department are not being maintained. This is a repeat item of noncompliance.

This is a Severity Level VI violation (Supplement VII).

License Condition No. 14 requires that licei aaterial be possessed and used in accordance with statements, rep oentations, and procedures contained in application dated December 10, 1976, and letter dated March 25, 1977.

Letter dated March 25, 1977, Item No. 2E states the Mediac dose cali-brator will be checked annually and a log will be kept of the recults.

Contrary to the above, it was determined through statements by licensee representatives and the NRC inspector's review of dose cali-brator records that this condition is not being met.

Specifically, as of the date of this inspection, linearity tests on the Mediac dose calibrator have not been performed.

In addition, records showing the results of dose calibrator accuracy tests were not available for review.

This is a Severity Level V violation (Supplement VII).

Appendix d' Pursuant'to the provisions of-10 CFR 2.201, you'are required to submit to this office within twenty-five days of.the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under. oath or affirmation.
