This refers to the routine safety inspection conducted by Mr. J. R. Mullauer of this office on April 16, 1981, of activities authorized by NRC Byproduct Material License No. 12-14860-01 and to the discussion of our findings with you and Prafulla Nagarija at the conclusion of the inspection.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
In addition to the above areas, the inspector exar.ined the actions described in your letter dated July 3,1978, regarding two apparent items of noncom-pliance found during our May 16, 1978, inspection of License No. 12-14860-01.
Your letter described the action which would be taken to avoid further noncompliance; however, during the course of this inspection our inspector identified one repeat item of noncompliance as specified in the attached Appendix A, Item No. 1.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
