APPENDIX A NOTICE OF VIOLATION Carolina Power and Light Company License No. DPR-62 Brunswick Unit 2 Based on the NRC inspection September 1-30, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indi-cated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
Technical Specification Paragraph 6.8.1 requires, that written procedures shall be established, implemented, and maintained covering applicable procedures recommended in Regulatory Guide 1.33, November,1972.
Contrary to the above Technical Specification requirement, an adequate procedure for completing and verifying required valve alignment for safety i
related systems had not been implemented, in that on September 7, 1980, it was found that on valve lineup verification documentation, locked valve PT l
46.1 for the RCIC and HPCI turbine exhaust manual stop check valve, that both valves were verified to be open, when in fact both valves were closed.
