Appendix A NOTICE OF VIOLATION Consumers Power Company Docket No. 50-330 Based on the inspection conducted on September 23 - 25, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. This item is an infraction.

10 CFR 50, Appendix B, Criterion X, states, in part that, "

If mandatory inspection hold points, which require witnessing or inscecting by the applicant's designated representative and beyond which work shall not proceed without the consent of its designated representative are required, the specific hold points shall be indicated in appropriate documents."

Consumers Power Company Quality Assurance Program Policy No. 10, Revision 9, paragraph 3.1, states, in part that, "

. These work activities were ac-complished according to approved procedures or instructions which include inspection hold points beyond which the work does not proceed until the in-spection is complete or written consent for bypassing the inspection has baen received from the organization authorized to perform the inspection Babcock and Wilcox Construction Company Procedure No. 9-QPP-102, Revision 7, paragraph 3.2.12 states, that, " Installation sequences identified as

" Hold loints" shall be initiated and dated by the organization (FQC-FWS-AI) establishing the " Hold Point" before work may proceed."

Contrary to the above, on September 24, 1980, the inspector established that the weld control record indicated that the Authorized Nuclear Inspector hold point on the Pressure Surge System Weld No. 3 on drawing No. M-6020 sheet I was not initiated and dated before the work proceeded. Furthermore, this weld was completed and the record was reviewed and accepted by Quality Control.

On September 25, 1980, the inspector determined that actions had been taken to correct this item and to prevent recurrence. Consequently, no reply to this noncompliance is required.
