Appendix A N,OTICE OF VIOLATION Memorial Community Hospital License No. 24-17680-01 As'a result of the inspection conducted on April 27, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),

the following violations were identified:

1.

License Condition No. 15' requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated July 11, 1977, and letter dated September 1, 1977.

The application dated July 11, 1977 states that the procedures de-scribed in Appendix C,Section VI of the NRC draft licensing guide shall be followed for calibration of the dose calibrator.

Appendix C,Section VI requires a daily constancy check on the technetium-99m setting of the dose calibrator.

Contrary to this requirement, it was learned through statements of licensee representatives and a review of records that daily constancy checks on,the technetium-99m setting of the dose calibrator were not performed.

This is a Severity Level V violation (Supplement VII).

Appendix C,Section II requires weekly wipe tests be performed.

Contrary to this requirement, it was learned through statements of licensee representatives and a review of records that weekly wipe tests have not been performed since the date cf license issuance.

This is a Severity Level V violation (Supplement VII).

Appendix A 3.

License Condition No. 15 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated July 11, 1977, and letter dated September 1,'1977.

The letter dated September 1,1977, states that the xenon-133 gas trap is checked for efficiency each week when the trap is used.

Contrary to this requirement, it was learned through statements of

-licensee representatives and a review of records that the xenon-133 gas trap was not checked for efficiency each week the trap was used.

Specifically, the trap was not checked for efficiency on sixteen occasions between August 11, 1979, and January 15, 1981.

This is a Severity Level V violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, thi: response shall be submitted under oath or affirmation.
