O l'O Appendix A NOTICE OF VIOLATIO" E:. Mary's Health Center License No. 24-17477-01 As a result of the inspection conducted on April 30, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:

1.

License Condition No. 16 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in application dated March 31, 1977, and letters dated May 12, 1977, and June 15, 1977.

The license application received March 31, 1977, and the letter dated May 12, 1977, states that daily monitoring of all elution and preparation areas shall be performed and records of the survey results shall be maintained.

Contrary to this requirement, it was learned through statements of licensee representatives that records of the daily survey of all elution and prepara-tion areas were not maintained since the date of license issuance.

This is a Severity Level VI violat.lon (Supplement "II).

10 CFR 20.401(a) requires that you maintain records showing the radiation exposures on Form NRC-5, in accordance with the instructions contained in that form or.on a c! ear and legible record containing all the information required by Fcra NRr 5 Contrary to this requirement, your film badge records do not show the social security. numbers for persons involved in work with byproduct material.

This is a repeat Severity Level VI violation (Supplement IV).

l Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the~date of this Notice a written statement or explanation in reply, including for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
