This refers to the routine safety 1.Jpection conducted by Mr. W. P. Reichhold of this office on April 30, 1981, of activities aathorized by NRC Byproduct Material License No. 74-17477-01 and to the discussion of our findings with you and members of your staff at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to cocpliance with the Commission's rules and regulations and with the conditions of your license. The nspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
Tn addition to the above areas, the inspector examined the actions described in the letter signed by Larry P. Courter, M.D., dated September 27, 1978, regarding the apparent items of noncompliance found during our August 24, 1978, inspection. We have no further questions regarding Item No. 1 of the ".ugust 24, 1978, inspection.
Our inspector noted, however, that Item No. 2 of the August 24, 1978, inspection was not corrected as described in your letter dated September 28, 1978. Con-sequently, this item is identified again in Appendix A Item No. 2.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
