Framingham, Massachusetts 01701 License No. DPR-3 As a result of the' inspection conducted on March 1,1981 - March 31,1981, and in accordance with the Interim Enforcement o licy, 45 FR 66754 (October 7, o
1980), the followi.ng violation was identified:
10 CFR ' Appendix B, Criterion V, states in prxt, that; " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions,
~ procedures or drawings..."
YankeeAtomicElectricSpecifIcationforthemodificationofReactorSupport Structure Column Bases, YR 80-24-S1, Section 8.0, requires the contractor's Quality Assurance Program to comply with 10 CFR 50, Appendix B.
Contrary to the above, during January 1981, the work performed to remove the concrete cover over longitudinal reinforcing steel in the existing i.
column basemat was not performed under quality control procedures, instruc-tions or drawings prescribed to control concrete removal.
This resulted in injurious defects to the longitudinal reinforcement.
This is a Severity Level IV Violation.

Pursuant to the provisions of 10 CFR 2.201, Yankee Atomic Electric Company is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:) c(or)rectise 1 the corrective steps which have been taken and the results achieved; (2
steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affinnation.
