Dear Sir:
Subject : Response to Inspection Reference is made to I6E Inspection No. 50-29/81-05, which was conducted by your Mr. T. Foley on Ms.rch 1-31, 1981 at the Yankee Nuclear Power Station (Yankee Rowe) in Rowe, Massachusetts. The report made subsequent to that inspection identified one item which apparently was not conducted in full compliance with NRC requirements.
In accordance with Section 2.201 of the NRC's " Rules and Practices," Part 2 Title 10 Code of Federal Regulations, we hereby submit the following information.
Alleged Violations 10 CFR Appendix B, Criterion V, states in part, that; " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings... and shall be accomplisned in accordance with these instructions,
procedures or drawings..."
Yankee Atomic Electric Specification for the modification of Reactor Support Structure Column Bases, YR 80-24-S1, Section 8.0, requires the contractor's Quality Assurance Program tra comply with 10 CFR 50, Appendix B.
" Contrary to the above, during January 1981, the work performed to remove the concrete cover over longitudinal reinforcing steel in the existing column basemat was not performed under quality control procedures, instructions or drawings prescribed to control concrete removal. This resulted in injurious defects to the longdtudinal reinforcements."
Response
During the removal of the concrete cover on the reactor. support column footings with jack hammers, some of the reinforcing steel bars

Note 4 on the construction drawing identified this precaution. The
. cognizant plant individual, after being made aware of the strikes, performed a visual examination and did not feel that an.immediate evaluation was necessary.
However during the Reference (b) inspection, the inspector felt the concrete

removal was not performed using approved procedures, instructions or drawings a'nd that an. immediate evaluation of the " injurious defects to the longitudinal reinforcement" was necessary.
Corrective Step Taken and Results Achieved Based on the inspector's concern, an immediate evaluation of the reinforcing steel was conducted which reinforced our previous finding that no' problem
- existed from a structural standpoint. The nicked edges of the rebar were grouad smooth to decrease the affect of stress risers.
' Corrective Steps Taken to Avoid Further Violations In the future, specifications and instructions will be reviewed to insure that sufficient detail is provided to contractors and plant personnel such that the likelihood of future occurrences will be minimized.
