REPORT NO. 1 (IKTERIM)
On'May 20, 1981 TVA informed NRC-0IE, Region II Inspector Pete Taylor of a potentially reportable condition under 10 CFR 50.55(e) regarding QA documentation for the drywell framed embedments. This is the first interim report on this condition. A more detailed report will be provided by October 30, 1981.
Description of Deficiency The contract with the manufacturer, Atlas Machine and Iron Works, for the drywell framed embedments was cancelled for lack of performe.nce. Frames were removed from Atlas shops under various stages of fabrication.
Frames 10 and 11 were almost complete when .emoved from Atlas. TVA made the decision to finir5 them at Phipps B.nd site by TVA CONST.
Upon receipt of QA documentation under court injunction, many discrepancies were discovered. It is anticipated that other frames originated under the same contract will have similar problems.
Additional noncenformance reports will be written on these frames as required. ,
Corrective Action TVA is presently evaluating all available documentation for the frames.
Where it is possible, TV4 is redoing NDE using CONST site-approved proeidures and personnel in order to obtain some level of confidence in Atlas NDE documentation. TVA is still in the process of evaluating the i subject deficiencies.
