Identification and Description of Occurrence:

During EMI-8, equipment airlock. doors 237 and 238 were opened simultaneously.

Conditions Prior to Occurrence:

Unit i refueling outage Unit 2 at 991 Unic 3 at 99%

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

Immediately closed doors to reestablish containment as required in T.S. 3.7.C.1, realigned the solen,oid.

Apparent Cause of Occurrence:

Electric strike on door 238 failed allowing doors 237 and 238 to be opened simultaneously. Failure due to misalignment of solenoid.

Analysis of Occurrence:

There was no danger to the health or safety of the public, no ' release of activity, no damage to the, plant or equipment, and no resulting significant chain of events.

Corrective Action

Solenoid properly aligned, successfully tested, and returned to service.
