PHIPPS BEND NUCLEAR PLANT UNIT 1 - REPORTABLE DEFICIENCY - RPV SHROUD HEAD AND MOISTURE SEPARATOR ASSEMBLY PIRE (NCR PBNP-106)

Initial notification of the subject deficiency was made to NRC-OIE, Region II, Inspector R. W. Wright on June 13, 1980. ne final report was tubmitted on December 19, 1980. Our corrective actions were not completed by the date specified in the final report as discussed with P. A. Taylor of your staff en May 11, 1981. Enclosed is our revised date for completion of corrective actions in addition to recurrence prevention information requested by Region II Inspector B. R. Crowley in Inspection Report Nos. 50-553,

a PHIPPS BEND NUCLEAR PLANT UNIT 1 ACTOR PRESSURE VESSEL SHROUD HEAD AND MOISTURE SEPARATOR ASSEMBLY FIRE 10CFR50.55(e) REPORT NO. 4 SUPPLEMENT AND REVISION TO FINAL NCR PBNP-106 As discussed with NRC-0IE Inspector, Pete Taylor, on May 11, 1981, the TVA commitment (in my letter to you of December 19, 1980) to complete corrective actions on the subject assembly has not been met. Cleanup work is now approximately 50 percent complete. TVA will complete the remaining corrective actions by July 31, 1981.

Also, as verbally requested by B. R. Crowley in his exit interview at Phipps Bend on January 23, 1981, below we indicate VSL actions taken to prevent recurrence:

VSL has prepared procedure-PB-27-R2, " Shroud Head Offloading," to prevent future fires and the procedure has been approved by TVA.

This procedure includes the following precautions:

1. VSL will place an approved fireproof material between the guide T* i lug and the shipping container brace / bracket during removal of ,!

the guide lug.

2. VSL will have a standby water pump ready during the removal of guide lugs.

3 A fire watch will be present during guide lug removal consisting of an individual with a charged fire extinguisher.

4. VSL will require verification that the RPV moisture separator packaging material complies with ANSI N45.2.2.

Upon completion of corrective actions, TVA will submit the summary

results of the assembly cleanup program as verbally requested by F. S.
