PHIPPS BEND NUCLEAR PLANT UNIT 1 - REPORTABLE DEFICIENCY - OMITTED REBAR FROM SHIELD WALL - PBF.D-50-553/81-09 The subject deficiency was initially repcrted to NRC-0IE, Region II, Inspecter Pete Taylcr en February 27, 1951, as NCR P?NP-197. In ccapliance with paragraph 50.55(e) of 10 CFR Part 50, we are enclesing the first interim report on the subject deficiency. We consider 10 CFR Part 21 applicable to this nonconfer=ance. ENCL'OS'URE PHIPPS BEND NUCLEAR PLANT. UNIT 1 OMITTED REBAR FROM UNIT 1 SHIELD WALL PBRD-50-553/81-09 REPORT NO. 1 (INTERIM)-
Description of Deficiency Beinforcing bars shown on C. F. Braun drawings Y-010 and Y-013 between Azimuth 105 and 1?50 were omitted'from.the reactor _ shield building.
The design drawings were not consistent between elevations. Design drawing Y-010 shows rebars terminating at an elevation 5'-2", and design drawing Y-013 shows~rebars cadwelded at an elevation of 15'-2".
The apparent cause of thia deficiency was a drawing discrepancy between-design drawings. TVA inspectors, however, did not identify the discrepancy and the rebar was'placed.and inspected in accordance with drawing' Y-010','
Corrective Actions TVA will drill h' oles in the ceactor shield wall and grout in 16'-9"-long splice bars (44 #11AW), 6'-9" below elevation 5'-2".
A rebar verification group has.been instituted to work directly with the construction trade forces during rebar installation'.' This group is structured within the project engineering /ci.vil unit and has the function to guide construction personnel and correct. discrepancies before they become a problem of complex nature or have reached a stage of nonconformance. Verification before final inspection should eliminate errors caused within the rebar, structure. TVA.has notified GE/C. F. Braun of this nonconforming condition and has requested they.
provide steps to be taken to prevent recurrence'of similar drawing errors.
