Inspection on January 6-8, 1981 (Report No. 50-358/81-03)

Areas Inspected: Review of preservice inspection (PSI) procedures, work

activities, nondestructive examination (NDE) personnel c-rtification and

data; IE Bulletin 80-08 activities. The inspection involved a total of

15 inspector-hours onsite by one NRC inspector.

Results: Of the areas inspected, one apparent item of noncompliance was

identified (Severity Level V Violation - failure to follow established

SNT-TC-1A, 1975 Edition ~. The inspector reviewed Kaiser procedure

" Nondestructive Examination - Personnel Qualification No. 45", and found

that.this procedure is nowJin accordance with SNT-TC-1A, 1975 Edition.

.(Open) Unresolved Item (358/80-20-02): No specific time for a QC inspector

-to be in the weld test shop. The site was working on the time to be spent

in the weld test. shop and how it could be incorporated in the procedure

"QC-Duties.in the Weld Test Shop".

The following discrepancies were found:

(1) Weld HP-16. The film package has line #1HP03A14 and the

radiographic (RT) report has HP08C3.

It was not known

which is the correct line number.

-(2) Weld FC-46B. In June 1977 there was a repair made and one

area was radiographed as an information shot.

In June 1980

.the entire weld was re-radiographed again with the repair

area not being stated as an R1 on the radiograph or stated

as a repair on the June 1980 RT report.

(3) 1 Weld HP-15. -The-original RT report states that three areas

are to be, repaired but there are no original radiographs.

There is another.RT report that states five areas are

repaired along with radiographs.

(4) Weld FWK-31. There are penetrameters in the area of interest

across welds in all radiographs taken.

The inspector made an inspection on November 8-9, and

December 12-13, 1978 (Report No. 358/78-30) and found several

discrepancies in radiographic techniques and reports that resulted

in an item of noncompliance. During a September 1979 inspection

(Report No. 358/79-17), the inspector found several more dis-

crepancies during his review of licensee actions. The licensee

then hired NES to review radiographs and reports for turnover

prior to operation.

NES started the review October 1979 and

completed April 1980. Radiographs of 2390 welds were reviewed,

958 radiographic reports had discrepancies, 543 welds had

unacceptable radiographic technique problems and 14 welde had

defects.

The licensee informed the inspector that the above 20 welds are

part of the 2390 welds that had received a final review by NES.

The discrepancies identified during the inspector's review of

the above field weld packages are considered a repeat item of

noncompliance (358/81-03-01).

b.

A Henry J. Kaiser Company Radiographic Report Form has been used

since radiography has been performed on site.

In the lower left

hand corner of this form, there is a block stating "MTL Approval"

(Magnaflux Testing Lab). This block was used for MTL approval

while the lab was on site and the MTL approval block continued

being used when NES arrived on site. NES is not stated any place

on the radiographic report form. On the radiographic film, NES

is part of the identification. The licensee informed the inspector

that all forms that have been used by NES will be changed from MTL

to NES and all future forms will state NES. This is an unresolved

item. (358/81-03-02)
