Appendix A NOTICE OF VIOI.ATION Cincinnati Gas and Electric Company Docket No 50-358 As a result of the inspection conducted on January 6-8, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion IX states in part that, " Measures shall be established to assure that special processes, including.
. nonde-structive testing, are controlled and accomplished by qualified personnel using qualified procedures in accordance with applicable codes, standards, specifications, criteria, and other special requirements."
un. H. Zimmer Nuclear Power Station FSAR, Chapter 17, Paragraph 17.1.9.2
- states "Special processes are accomplished and controlled by qualified personnel using qualified procedures in accordance with applicable codes, standards, specifications, criteria, or other special requirements."
Chapter 3, Section 3.2 of the FSAR classifies systems accruding to the safety function they perform and specifies the applicable Code for each system.
Contrary to the above and ASME Section III, 1971 Edition (Code specified in FSAR Section 3.2 for the radiographs and reports reviewed) Article NA-4000, unacceptable radiographic techniques and discrepancies in Kaiser Radiographic Reports were observed in four of the twenty field weld docu-mentation packages reviewed.
This is a Severity Level V violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation -in reply, including-for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncomplic.oce; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amende/,, this response shall be submitted under oath or affirmation.
