APPENDIX A NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Haddam Neck Plant License No. DPR-61 Docket No. 50-213 As a result of the inspection conducted on November 19-21, 1980 and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980) the following violation was identified:
Title 10, Code of Federal Regulations, Part 50, Appendix B, states, in part:
"XI.
Test Control...
Test results shall be documented and evaluated to assure that test requirements have been satisfied."
Contrary to the above, the following three examples of inadequate evalua-tion of test results were found:
1.
Incorrect, algebraic end point correction to the reactivity worth of rod B-31 with Control Bank B inserted.
2.
No review or approval signatures for procedure SUR 5.3-6, Control Rod Group Reactivity Worth-Combined with Boron Worth and Ejected Rod Worth, performed July 25-26, 1980.
3.
Numerous errors in data analysis of moderator temperature coefficient.
This is a Severity Level V Violation. (Supplement I)
Pursuant to the provisions of 10 CFR 2.201, Yankee Atomic Power Company is t.ereby required to submit to this office within twenty-five days of the date of this notice, a written statement or explanation in reply, includin (1) the corrective steps which have been taken and the results achieved; (g:2) corrective steps which will be taken to avoid further violations and; (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as-amended, this response shall be submitted under oath or affirmation.
