Appendix A NOTICE OF VIOLATION Cincinnati Gas and Electric Company Docket No. 50-358 As a result of the inspection conducted on January 5-30, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (Oc*.ober 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion XI requires in part:
"A test program shall be established...and performed in accordance with written test procedures.. "
Wm. H. Zimmer FSAR, Section 14.1.1.2 requires in part:
"All preopera-tional... tests will be conducted in accordance with approved written test procedures, administrative controls and supporting administrative procedures..."
Contrary to the above, during the performance of Preoperational Test P0.RD.02, Control Rod Drive Hydraulic Preoperational Test, the following examples of failure to follow procedure occurred:
1.
A step was added to the procedure (between 7.4.2.5 and 7.4.2.6) without use of a Test Change Notice.
2.
A note after step 7.4.2.6 (which required valve IC11-D001-F107 remain closed) was not followed. When step 7.4.2.7 was performed, water was blown onto the floor and personnel performing the test.
3.
Test section 7.4.1 and 7.4.1.54 were not verified as being completed.
4.
The instrument used to record acceptance criteria (1C11-R611) was nine months overdue for calibration.
This is'a Severity Level V Violation.
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance vill be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath
