k MAINE YANKEE ATOMIC POWER COMPANY United States Nuclear Regulatory Commission April 7, 1981 Attention: Mr. Boyce H. Grier, Director Contrary to the above, the six level switches for the safeguard pumps suction Recirculation Actuation Signal have been installed in an operable condition and in a hold status since December 31, 1980 without approval of the resolution of their non-conformity.

These items of noncompliance collectively represent a Severity Level V Violation (Supplement I.E.).

RESPONSE

f A.

Investigation into this item of noncompliance reveals the following information.

1.

The hold tag on the motor operator on safeguard valve HSI-M-50 was placed on the valve for information only. No non-comformance existed, although hold tags should not be used for this purpose.

2.

The non-conformity for the six level switches was reviewed by PORC on 1/15/81, by YNSD on 2/23/81 and was inadvertently delayed in routing to the Manager of Operations for approval. Actions recommended in this non-comformance have been carried out and the switches will be i

replaced.during the upcoming refueling outage.

ITEM OF NONCOMPLIANCE B.

Technical Specification 5.8.1 states in part, " Written procedures shall be established, implemented and maintained covering...a.

The applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33."

Regulatory Guide 1.33, Appendix "A" states in part, "The following are... safety-related activities that should be covered by written procedures...l.j. Bypass of Safety Functions and Jumper Control."

Contrary to the above, on February 3, 1981, the licensee failed to implement his Operation Safeguard Procedure prior to blocking the Engineered Safeguards Features Recirculation Actuation Signal "B" train for maintenance.

This is a Severity Level V Violation (Supplement I.E.).

RESPONSE

B.

Investigation into this occurrance reveals the following information:

1.

This event resulted from a Technicians failure to follow the requirements of Plant Procedure 16.1.

The individuals involved have been reindoctrinated in the use and content of the tagging and control procedures.
