NOTICE OF VIOLATION Commonwealth Edison Company-Docket No. 50-154 Docket No. 50-455 i
As a result.of the inspection conducted on March 19, 25 and 26, 1981, and-
'in'accordance with Interim Enforcement Policy 45 FR 66754 (October 7, 1980),
the following violation was identified:
10 CFR 50, Appendix.B. Criterion IX, states.in part that, " Measures shall.
r be established to assure that special processes including....... and non-destructive testing', are controlled........using qualified procedures in accordance with applicable codes, standards....... and other special requirements."

i ASME Code Section V, 1974 Edition,' Summer 1975 Addenda, which is the applic--
able Code Feverning radiography for the Byron Station primary coolant system states in paragraph T-262.3(b) "If more than one penetrameter is used, one shall be in the lightest area.of the radiograph and the other.in the darkest.
Paragraph T-262.4 states "If the weld reinforcement' and/or backing strip are not removed, a shim of material radiographically similar to the weld metal shall be placed under the penetrameter." Pittsburgh Testing Laboratory (PTL)'-
procedure QC-RT-1 governs this work, was approved by the -licensee, and reads exactly as the Code.
Contrary to the above requirements, the weld reinforcement had not been removed on Weld No. FW-45 in spool piece IH-CAE-4 and the radiographic:
I film did not indicate a shim was used under either of the two penetrameters i-shown.
,This is Severity Level V violation (Supplement II).
I Pursuant to the provisions of 10 CFR 2.201, you are required.to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, incitding for each item of noncompliance:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the-authority of Section 182 of the Ataic Energy Act of 1954, as amended, this response shall be submitted under oath-or affirmation.
