10 CFR 50, Appendix B, Criterion V as implemented by Topical Report Duke-1-A, Section 17, paragraph 17.1.5 requires in part, instructions, j

procedures or drawings shall include appropriate quantitative acceptance criteria for determing that important activitier have been satisfac-torily accomplished. The licensee's Radiography Procedure, NDE-10 speci-fies that welds that are shown by radiography to have discontinuities (slag inclusions) greater than the length permitted herein are unacceptable.

Contrary to the above, unacceptable slag inclusions shown on radiographic films in weld joint W-6, ISO 19A-25 were not rejected.

Response

System 19A, which is partially shown on ISO 19A-25 is the intake air line to the diesel engines in the Standby Shutdown Facility.

This line is classified as Duke Class C and sees only atmospheric pressure.

l The radiographic film for weld joint 6, view 4-5 on ISO 19A-25, showed indications which were read by the radiographer to the concavity. When questioned by the NRC inspector, the radiographer agreed to have the weld ground within the areas of view 4-5.

This was done and some slag was found.

Duke Power concurs that a violation did occur, and it resulted from an error in interpretation by the radiographer in reading the film for view 4-5 of weld joint 6 on ISO 19A-25. While the slag inclusion is felt 4

to be of minor importance in this instance, the weld was repaired and re-radiographed with acceptable results. The matter of the error in inter-pretation was discussed with the radiographer by supervision. Additional emphasis on slag interpretation will be incorporated with the training program for Duke Power radiographers, and the radiographer in question will be retrained in this area. This will be accomplished by June 1, 1981.
