Identification and Description of Occurrence:

Reactor zone ventilation isolation was bypassed.

Conditbns Prior to Occurrence:

Unit 1 at 95%

Unit 2 at 100%

Unit 3 at 100%

i Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

Checked RM's - they were all normal. Checked circuits, 16AK61A burned up.

(Started down) TACF to get R.B. vent back to prevent a hard scram.

r-t Apparent Cause of Occurrence:

While replacing relay, temporary alteration was employed which bypassed isolation.

Analysis of Occurrence:

There was no damage to plant equipment. There was no activity release, no personnel exposure or injury and no dange.r to the health or safety of the public.

Corrective Action

Unusual event initiated controlled shutdown.

IP 2 was initiated, repairs

were made and upon completion of repairs and return of the ventilation system to nor=al, the shutdown was terminated.
