99900256/80-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on December 8-11, 1980, it appears that certain of your activities were not conducted in accordance with the NRC requirements as indicated belcw:
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructicns, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviation from these requirements is as follows:
Paragraph 9.3.3 of the ASME accepted QA Manual requires welders to have eye examinations on a periodic basis.
Contrary to the above, two (2) welders eye examination records had expired.
However, the inspector verified corrective actions in that all components where the two welders had welded were rejected until eye examinaticns were success-fully passed.
In addition, a policy change was issued to prevent recurrence.
No response to this deviation is necessary.
