At 0600 while reducing power due to a saltwater leak, the condensata 4

demineralizers were placed in service and the ammonia pump was started.

i During this time the pH level decreased to 6.6 belcw the Tech Spec limit

{

of 7.6 (T.S. 3.7.1.6.b). The cemineralizers were imediately taken out

{

of service and a valve from the ammonia tank to the manifold was discovered to be closed. The valve was opened, and pH returned to normal i

at 0820. This is not a repetitive occurrence.

CAUSE DESCRIPTI0ft AND CORRECTIVE ACTIOilS:

A Lab Technician when placing the ammonia feed pump in service, f ailed to notice that the feed line from the amonia tank to tne manifold i

header had been shut off. Corrective action was initiated at approxi-I i

mately 0700 by opening tne manifold valve to allow ammonia feed. All i

Rad-Chen Technicians were reinstructed in the proper valve operation
