& NPF-7 Based on the NRC inspection June 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.

License No. NPF-4 Technical Specification 3.3.1.1 Table 3.3-1 for Steam Generator Water Level Low-Low and Low Steam Generator Water Level Note 7 l
requires that if the number of operable channels is one less than the total number of channels, startup and power operations may proceed until the next required Channel Functional Test provided the inoperable channel is placed in the tripped condition within one hour.
Contra ry to the above, on June 12, 1980, while operating in Mode 1 the 2

inoperable Steam Generator Level channel was not placed in the tripped j
condition within one hour.

License No. NPF-7 Technical Specification 6.8.1.a requires that procedures be developed, implemented and maintained for the procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978. Section 3 4
of Appendix A discusses procedures for Startup, Operations and Shutdown of l
Safety Related Systems.
Contrary to the above, 2-0P-1A " Pre Startup Checkoff List" had not been revised to require the checking of instrument stop valves in order to insure Reactor Protection System instrumentation was in service prior to entering the modes whert it would be required. Specifically, on June 19, 1980, while in Mode 2, flow transmitters FT-2476, FT-2477, FT-2487, FT-2496, and FT-2497 were found to be isolated.
This is an infraction.

Feedwater Flow Mismatch Note 7 requires that if the number of operable channels, is one less than the total number of channels, Startup and Power Operations may proceed until the next required Channel Functional Test, provided the inoperable channel is placed in the tripped condition within 4

Additionally, Technical Specification 3.0.3 states that if a Limiting Condition f r Operation and/or associated action requirements cannot be satisfied '. acause of circumstances in excess of those addressed 4
in the specification, the unit shall be placed in at least Hot Standby with-in one hour and Hot Shutdown within the next six hours.
