Based on the results of the NRC inspection conducted July 1-31, 1980, it l
appears that certain of your activities were not conducted in full com-l pliance with NRC requirements, as noted below. These items are infractions.

10 CFR 50, Appendix B, Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures... and shall be accomplished in accordance with these instructions, procedures.
Sargent and Lundy technical specifications for concrete work K2944 and 17, February 15, 1980 and CPS-1-MCS specification for making

and inspecting mechanical (cadweld) splices, Revision 9, December 7, 1979 state that work shall be performed per the latest edition of the manufacturers instructions (Erico).
J Contrary to the above, 250 containment rebar cadwelds were installed (fired) out of their normal position and without following the instal-lation instructions denoted in the Erico Catalog and Addenda of 1978.

report disposition from Baldwin, a site job ticket is prepared, dispositioned, reviewed and approved by the site Superintendent and QC Supervisor and issued to the field for work."
Contrary to the above, 38 safety-related HVAC hangers in the 800' level of the Control Building were installed incorrectly and in disagreement with the disposition instructions of Nonconformance Report (NCR) 3030.

The Zack Co. (HVAC) Site Installation Procedure CB-QCP-15 paragraphs 5.4 and 5.4.4 state in part, ".
. site QC is responsible to perform and document all inspections as required by the installation job ticket site QC is responsible for issue, control of NCR item, and administration of the NCR.
