- NOTICE OF VIOLATION Veterans Administration License No. 22-01859-01 Medical Center As a result of the inspection conducted on May 14, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the folicwing violations were identified:

License Condition No. 23 states that licensed material shall be used l
in accordance with statements, representations and procedures contained l
in letter dated August 7, 1979, and other referenced documents.
l Item No. 4 of that letter specifies monthly smears for removable l
contamination be performed on a monthly basis.
Contrary to the above, monthly contamination surveys were not performed in Building 54, Room 8, a research lab where byproduct material was

in use, for the months of January and February 1981, January, February, March, May, and August 1980 and other months.
