Dear Mr. O'Reilly:
As defined by Technical Specifications for Connecticut Yankee Atomic Power Station Section 5.3, the following information involving an unsatisfactory test of one of four main steam isolation valves is hereby reported.
In accordance with Section 4.9 of the CY Technical Specification the Main Steam Isolation Valves are tested each week for a movement of approximately one and one-half inches. The #3 Main Steam Isolation Valve operated satisfactorily on its first test actuation however, when the second test actuations were made, it failed to operate.
The valve was tested subsequently and operated satisfactorily. The other three valves test operated satisfactorily.
A follow up report will be submitted in ten days.
