Dear Mr. O'Reilly:
As defined by Technical Specifications for Connecticut Yankee Atomic Power Station, Section 5.3, the following information involving a Reactor Coolant System leak is hereby reported.
At 0530 on March 26,1975, with the plant operating at 100% power, a high temperature alarm was received on the valve stem leak off outlet from the valve steam leak off cooler.
Aninvestigationwasstartedandanentryindothecontainment revealed that the cause was a leaking valve stem with-in the Reactor Coolant Loop area. At 1300 an alarm was received in the MCR on the Volume Control Tank Level " Rate of Change" followed shortly by an alarm on the Containment Air Particulate. An increase in the containment sump level was observed and at 1302 an orderly Plant Shutdown was commended. A Containment Entry indicated the valve stem leak off system relief valve had lifted relieving to the containment At 1445 the Plant Shutdown completed, and at 1638 a containment sump.
entry was made and it was determined that LD-M0V-200 was the source.
A plant cooldown was commenced and at 0630 on March 27, 1975 with the RCS at 3850F and 550 PSIG. LD-MOV-200 was repacked with it in its backseat position.
A final report will be submitted within ten days.
