Based on the results of an NRC inspection conducted on October 6-7, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states in part; " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures.or drawings.
Section 4.0 of your QA Manual states in part;
...the customer's orders are correctly translated into specifications, drawings, procedures, or instructions to satisfy ASME Code and contractual requirements.... "
4.2.1 "For handling Nuclear Orders;" assign the responsibility to ensure that the ASME Code and contractual requirements are identified and docu-mented, by the Specification and Design Review Committee, and Reported to Design and Manufacturing Engineering, to be appropriately incorporated into pertinent drawings and process sheets.
Contrary to the above, the Specfication Design Review Report to Manufac-turing Engineering failed to identify a code requirement, that PWHT was madatory after welding SB-148 alloy 954 material, which resulted in nine valve discs manufactured from SB-148-954 and requiring certain repairs made by welding not being PWHT in compliance with the code.
