O APPENDIX A NOTICE OF VIOLATION Maine Yankee Atomic Power Company Docket No. 50-309 Based on the results of an NRC inspection conducted on October 6-31, 1980, it appears that certain of your activities were not conducted in full com-pliance with the conditions of your NRC Facility License No. DPR-36, as indicated below.
Item A is categorized as an infraction, item B as a deficiency.
A.
10 CFR 50, Appendix B, Criterion V, states in part, " Activities affecting quality shall be prescribed by documented... procedures...
and shall be accomplished in accordance with these... procedures".
The facility Operational Quality Assurance Program states in part,
" Compliance with requirements of the... Program - based on the cri-teria of Title 10 of the Code of Federal Regulations, Part 50, Appendix B, and ANSI N18.7 - 1976 - shall be the responsibility of all personnel ANSI N18.7 - 1976 states in part, "5.22. Procedure Adherence.
Procedures shall be followed...".
Procedure 9.1.15, " Shipment of Radioactive Material", requires in part that, "7.3.2 Quality Control personnel will verify that the handling equipment used to load radio-active material has been inspected, checked, and is designed to handle the intended load weight".
Contrary to the above, during an inspection of the loading process.
for radioactive waste shipment 10-80-069A en October 6, 1980 the inspector observed that the cask lid and full liner were lifted with-out the above required verification.
B.

Procedure 5-141, " Inspection and Maintenance of Turbine Hall and Yard Cranes", states in part, "This procedure shall be completed prior to a refueling period..
7.1 Complete the crane inspection report".
Contrary to the above, prior to the January 1980 refueling period, the crane inspection repcrt was not completed for the yard crane in accordance with this procedure.
